Gravatar You left out an important part of the team: the patient. Patient population has a HUGE impact on outcomes (low income/multiple chronic medical conditions/low education level/substance abuse/etc). Individual patients can have a huge impact on their own care. Patients will do things you tell them not to (nothing to eat or drink after midnight Mr. Jones!) and won't do things you tell them to do (Turn, cough, and deep breathe Mr. Jones!). I have had family members decide to remove post-op dressings even after being instructed to leave them alone because "I needed to clean that up"--uh, no, you didn't. I have found patients with NG tubes sucking down fried chicken that the family brought them because "y'all were starving daddy".

Of course the medical team is extremely important in taking care of patients--that's the purpose of the medical team! But without the cooperation of the patient, the best team can have huge numbers of problems.
