I broke my wrist a few weeks ago. Went right to the emergency room, got fixed up fine. Never signed anything, and will never get a bill. I go to my family doctor about four times a year for a little troublesome thing I have (no biggie). Again, never sign anything, never see a bill.

My typical American healthcare dilemma: Not only do I have two serious chronic conditions (high blood pressure and diabetes) I also have a scabby lesion on my right hand that could be melanoma, but because I'm essentially indigent, I sit and worry and can't go to the doc to find out or get treatment.

Here you'd pay HK$45 (roughly US$6) to go for a clinic visit, where they'd take your blood sugar and BP and the doctor would discuss with you about any recent problems (like the skin issue) and then write you a prescription to get filled at the clinic dispensary and tell you to come back next month.
in the case of a crisis event, they rush you to the ER and you pay HK$100 (~US$13) for ambulance, ER medicine, bed for the night (not incl. dinner) if necessary, etc... and after you're stabilised, tell you to go see the clinic for a follow up a week or so later.

The diabetes I control with a low-priced generic sulfa, testing 3 times a day, and diet, for now. The HBP with a trio of of medicines, all off which are now available as generics. Still, making ends meet is really tough.
There prolly is a free clinic in Portland, I shall have to check.

As it still is. The only reason they're changing policies that they vigorously defended up to the very second that the judgment went against them is that it has suddenly become too expensive to do it the old way.
Why are we belaboring the obvious? The reason for-profit insurance companies are in business is for profit.
They'll think of some other way to fleece their suckers, err, customers and keep the profits rolling in, just not this way.
