It can be helpful to think of how each new proposal works towards those each goals and how trying for one reduces the chances of getting the others.

Single payer is the way to go but as in anything the devil is in the details. Since employment based plans are generally annual contracts with different dates - for the company I work for our annual enrollment for benefits runs 9/1 through 8/31. For my wife it is 10/1 through 9/30. So if we end up with a Single Payer plan that goes into effect 10/1/10 what do I do for the month of September 10? Jump on my wife's plan for one month, go uninsured one month or count on Kaiser (my current provider) to sell my boss a one month policy for all of his employees?
This may seem like a trivial question but in Obama's comments this week about "too radical" of a plan he seemed to be worried about this important issue. Anyone have any thoughts in regard to a transition from private to public plans?

Earlier today an Israeli Navy team boarded and took control of the cargo boat 'Arion' as the vessel illegally attempted to enter the Gaza Strip. In accordance with internationally recognized agreements between Israel and the Palestinian Authority, the waters along the Gaza Strip are under Israeli security control and foreign vessels are not permitted to sail into the territory. The Israeli Navy attempted to make contact with the 'Arion,' but its repeated warnings were ignored. No shots were fired during the boarding of the boat and the vessel was rerouted to the Israeli port of Ashdod. Humanitarian goods found aboard to boat will be transferred to the Gaza Strip, subject to authorization. We expect the vessel's passengers to be flown home shortly.

You buy a COBRA policy for that month.
COBRA is a supplemental insurance policy that provides temporary health insurance to eligible workers and their dependents when they are between jobs by extending the coverage of the most recent employer. Brooklyn Girl - nice try but COBRA is not an option that the vast majority of the country can afford even for one month. Also our current provider agrees to sell us insurance in one year contracts. If we were not renewing a contract but only looking to have employees buy coverage for one month COBRA would not even apply. Each employee would have to negotiate a rate on their own and many would be deemed un-insurable for individual coverage so they would not be able to pick up coverage at any price.
COBRA only works for loss of coverage for an individual when the group they used to belong to is still insured.

Dirk- I can see how that would work but what about this scenario:
My boss agrees to a new annual contract with Kaiser for 10/1/09 - 9/30/10 for group coverage. By some miracle my dreams come true and the nation institutes Single Payer Insurance for all effective 6/1/10. My taxes go up but I am happy because my boss will give me a raise nearly equal to what he used to pay for my health insurance. The problem is Kaiser won't let him out of his contract so we end up with double coverage for 3 months.
The bottom line is that these individual contracts for group insurance all have different date ranges but they will all need to terminate at the same time, somehow, in order to accommodate single payer - I am sure there is a way but it will be messy figuring it out.

no, he pays for the policy he contracted to pay for. kaiser pays for the care. look, it may not be 100% fair to every individual and some people may get a little benefit because of a gap and others may have to suck it up for a couple of months. compared to the 47 million that have been without any care at all for however long, i just don't think it's that big of a deal. it isn't like saying "gays have to pay extra" or even "republicans should pay more." it's non-discriminatory because it's essentially random.
if it's that big of a deal, give a tax credit to business that overlap, and charge an extra tax fee to those with a gap. it isn't really that complicated. not enough to stop the whole process dead in it's tracks.

OFA is a total Obama creation, one reason I don't work much with it, and it's been actively lobbying both Democratic and Republican reps for months. If he actually thought it was bad to apply pressure like that, why would Plouffe have been working on it full time.

let me be clear on what i meant earlier: the single payer insurance would not, on the day it took effect, relieve insurance companies of their obligations to pay claims as long as they were still charging premiums. so if kaiser wouldn't let the employer out of his contract, they wouldn't just get to keep all the money and not have to pay any claims. the money that they pay out would not also have to be paid by the gov't single payer plan, thus saving some of the universal pool, reducing costs for everyone.

We're all going to die, so this notion that somehow the government protects our lives indefinitely is a perversion of what Jefferson wrote. The issue is about equitable protection against unforeseen calamities. That's what insurance is, and that's what we should be aiming for with health care policy.
But I don't think we've even begun to have a proper conversation about what we think is a baseline of care and how that relates to quality of life.

Having said that, giving people incentives to get their weight down, stop smoking, exercise, eat well, etc. can only be a good thing.
My medical insurance reimburses a percentage of my health club membership, provided I go 50 times in a six month period and give them a verified written record of my visits. That's an additional incentive to go.

Those three words are beside each other in the same sentence because they all state the same thing and together contribute to the same meaning. Reading healthcare as 'right' into the single word life in that sentence is certainly an error.
The 'general welfare' is an abstract way of defining the role of government. That would include general things first, like public safety. In the 18th century context government sponsored healthcare would be a highly unique and specific idea, if not revolutionary. It's not what they meant.

Not sure if I understand you second paragraph, but seems like obesity can lead to many illnesses. Diabetes, high blood pressure, certain cancers--I can't remember them all right now.
Certainly, early intervention could help prevent some type 2 diabetes, but that means basic lifestyle changes, including eating habits (I'm looking at the food industry here) and exercise.

I dont believe that a 98 score on a Federal job means that there's no requirement to interview. I believe that the chanbge in fiscal year means that there will be an interview. A 98 along with a veterans 5 point bump? would require that the vet had a 93 - or a 103 point rating. Nevertheless - the list contains ME. And in that list there may well be some other folks, but their scores arent going to exempt me.

I was recently looking at racial differentials in life expectancy, when some shithead troll was babbling about black men shooting each other.
Turns out that 15% of the differential was attributable to hypertension, followed by diabetes, AIDS, and homicide. The first two of those are very much related to obesity, and the third one has more to do with education and poverty differentials.
Taking a country like Japan as a norm, you could first factor out transportation fatalities and homicidal violence differentials. Even if you pulled those out, there's still going to be a lifespan differential. Environmental factors like pollution are a wash, so it's going to come down to genes and lifestyle. They may have some genetic advantage, but the vast differences between their obesity rate and ours almost certainly explains most of the difference.
I am not saying in any way shape or form that the lack of universal coverage in the US doesn't have an effect, but rather pointing out we need to stop thinking how to take care of being ill and learning how to make our society healthy.
Prevention is almost always a much better strategy than reactive, intensive medical intervention. The fact that we have 50 million uninsured only exacerbates this, by giving people incentive to use emergency services (which are mandated as free to those who can't pay) and not go in for regular care (which isn't free, or is only available in a limited capacity).

I'd like to see a system that includes incentives for getting healthier, rather than one that punishes people for not taking better care of themselves. A strong, healthy body feels good.
But shit happens even to very healthy people. My mother was incredibly healthy and lived to be 88 ... except she should have lived a lot longer. She got a transfusion of tainted blood when she was 40, which gave her hepatitis, which in turn eventually caused the liver cancer that killed her. She lived 16 years longer than expected, and had a very high quality of life, because she took very good care of herself.

Actually, it's a charter of negative powers, with all liberties being positively reserved to the people. And one power granted to Congress? Article I, Section 8: "The Congress shall have Power To...provide for the...general Welfare of the United States." So that Britain could pay for the French and Indian War, American colonists were taxed. Probably more important was that American colonists were not allowed to move west of the Appalachins, to keep the peace with the Natives that fought for Britain.
Canadian settlers (at least the English ones) didn't have the issues at the time of the mid-1770s that the American colonists had. Hell, the Crown expanded Quebec down to the Ohio River valley just to piss us off.

That's about right. If you have a chance, I highly recommend the book '1491', which is a very readable overview of pre-European civilizations in the Americas. It was a real eye opener for me.
