When I read this it seemed to obvious to me why patients lie by omission to a therapist, but then I've only ever been in a patient role. I remember when I finally trusted my therapist enough to confide something to her that was really big, and I waited a long, long time to do so. She didn't seem the least bit surprised that I hadn't confided it earlier, and in fact her response was, "It's been my experience that clients often save the most difficult issues to deal with later in therapy." That was certainly the case for me. I wouldn't have been able to handle every single difficult issue all together near the beginning of therapy. I saved the most difficult, most painful stuff for last.
