A lot of time, configurability is needed to match up with a business's existing processes - IOW, since the customer isn't going to re-configure their work around your software, your software has to be configurable to fit their work.
I think a lot of "intuitiveness" isnt' really that, it's just learning and being used to a certain way of doing things. For example, I suspect experienced Mac users, experienced Windows users, and complete computer newbies have different ideas of what is intuitve.
That said, there definitely are superior user interfaces.

Actually in the Electronic design automation field their has been a very successful suite of tools based on a proprietary lisp dialect called SKILL.
i personnaly hate tools with a bolt-on command-line interface with not enough functionality to create batch jobs for what I want to do. i also hate slow GUIs that lock-up when processing a command.
