I'm guilty of all the faults you've listed, so I must be a rubbish friend too! Anyway, I don't think you're a rubbish friend - it's just that life is so full of stuff sometimes that it's hard to find the time/energy to keep in touch, you know what I mean...? Sometimes you just want to say hi - I haven't forgotten you but I'm just too busy/sad/distracted/whatever to say anything much, but that just seems abrupt or churlish, so you don't say anything (which is probably worse because then people think you have forgotten them...)
