I've thought that a vote in US elections a logical extension of the term "leader of the free world".
If the free world doesn't vote for the US President, then the US President isn't it's leader. If the US President leads nations that don't vote for the US President, then they aren't free.

And I suspect some occupied peoples might not be as apathetic about voting as many in the US apparently are. In the end, it's only a relatively tiny number of people who directly vote for a president/congress, who in turn decide how hegemonic interests will best be served.

Well, that's the point isn't it? The US doesn't have a popular democratic mandate for anything really.
