I would eat there, mostly out of curiousity though. And I'm sure most of the untensils and bowls they're using are fairly clean, otherwise they wouldn't use them.
As for origin, I'm leaning more toward Japan. Just judging from the characters (writing) that are in the background, but then again I'm no expert... c:

That's weird and I saw an equally crazy restaurant in a hospital setup. The chairs are wheelchairs and the utensils are obviously a mix of hospital tools and equipments. I guess they do okay for those who like to try what's new and something different but definitely not for the faint of heart.
