In the old total quality days there was a story called the Baked Ham Strategy that had a woman cutting off the ends of a ham. When she asked grandma what she always did that, grandma responded, "Because I didn't have a big enough pan." It was about "it's how I've always done it."
Second, in TQ there was a drive to eliminate opportunities for error. Writing the thing twice introduces huge opportunities for error. Excellent post.
