Yes they do get that large... and heavy... and tender and sore.... If you look close at her left boob, you can kinda see a pad that is made for absorbing leaks. The dress probably has a built in bra, but she is most likely wearing pads in case her milk comes down while she's out.

Just FYI-Not all women gain 1000lbs when pregnant and it does not mean that you were starving yourself either. Some people are naturally thin and others understand that the baby does NOT need that much xtra food. I cant tell you how many of my friends PIG out while pregnant and their OB has to tell them to calm the hell down. That said, its nothing wrong with carrying a little extra but you should dress appropriately if you want.
