Even the affluent of fairly industrialized countries will still fly to the US or England for healthcare (thats how Pinochet's ass got caught!) If Z really does have rickets, it can be cured with calciuim and vitamin D (& sun) in about a week or so. The family is in a good place for that. Just because they aren't in the US, doesn't make her a bad mother! Africa and most other countries have equivalent & someetimes better hospitals than America. Do you even have any idea how high the infant mortality rate is in the U.S.? The grand, mighty USA is only ranked 28th in terms of infant mortality! Babies have better chances of surviving past infancy in Slovenia (ranked 15), Singapore (ranked 2) and the Czech Republic (ranked 27).
If we were to compare state by state, many U.S. states would fare even worse. Statistically, a baby has a better chance of surviving its infancy in Cuba than it does in Washington State.

You can fly upto 9 days before your due dates if you get authorisation from your doctor and the airline, if you not rich and have the money to a gyn accompany you. You can fly whenever if you fly private with gyn sitting next to you.

Those of you bashing Namibia/Africa, have you ever been there?!?! I have! I was born and raised in South Africa. I now permanently live in the USA, have access to top notch medical care bec I have great insurance. Still, I feel I got better healthcare in South Africa. FYI - A South African doctor did the first successful heart transplant. I grew up in Cape Town, the only difference between it and Boston is the size. There are MANY modern countries in Africa with cities comparing to those here in the USA.
