"A couple more years in the ER" my ass. Stop making excuses and just do it. Don't let fear and insecurity stop you from doing something that you are obviously very interested in doing. Did you wait until you were perfectly prepared and ready to get married and have kids? Of course not. If you waited for such a time, it would never happen. This is not so different.

HI, Armed! Good to hear from you!
I know that the "couple of years more in the ER" thing sounds like a cop-out, but I really do need the extra couple of years. Here's why:
The airlift organization prefers CCU nursing experience in addition to 5 years of ER/Trauma nursing experience. I need to get that CCU experience, but I don't want to leave the ER.
So what I am doing is 1) continuing work in the ER; 2) gaining experience in CCU by working additional per diem shifts there; and 3) continuing to prepare for my Doctorate in Nursing Practice in order to teach at the University level.
Those 2 years are going to be busy indeed. But I look forward to it, and my wife, who has been so tremendously supportive and understanding for the last 18 years, simply laughs and says, "This is what God wants for you. Do it."
So I am doing it. It's just a busy process.
How goes with you? I hope you and yours are safe and well. Peace and grace to you, my dear brother and friend.

Yeah, well, OK. I guess that makes sense. I'd just hate to see you holding yourself back from some perceived sense of inadequacy.
Things are well with us, thanks for asking. This coming semester will be (Lord willing) my last in Bible College, and I'll graduate with my B.A. in Religious Ed.
...and then into the Paramedic program. Yep, after much prayer, counsel, and consideration I have decided to take the plunge and do the EMT-P course. The funny thing is that just over a year ago I was stressing out and worrying about whether or not I would be able to handle being an EMT. Like you, I am blessed and fortunate to have a supportive wife. She just shrugs and says, "I told you that you could do it."
Grace & peace.
