There are some other possibilities, too, that I mention over at my place.
They include confused voters (I had a lot of Dems try to vote at my polling place, and a several signed in and got access codes before the words "REPUBLICAN PRIMARY" penetrated their skulls. I canceled booths and crossed them out in the book -- but if I had missed crossing one out, they would be on the list as voting twice.
I'm also concerned about election judges who didn't mark their poll books or clerks who failed to heed the notations who then allowed an early voter to vote. Seems we need some accountability there, too.
