There is a 3rd possibility; duplicity. He certainly knew that a tried and true phrase of "lipstick on a pig" (many have used it, heck I use it all the time) worked especially well because of her previous comment about lipstick and pit-bulls.
The duplicitous part was that he did was play to a partisan crowd who "knew what he meant". He let them make the inference without correcting them and by doing so, he approved their prejudice.
He could have ended the "controversy" by correcting them then and there. It would have been mature and certainly the right thing to do.
Of course I gave up on the left doing the mature and right thing long ago.
