This tool is great for those unfortunate enough to have graduated and now find themselves slaving away every day for "The Man". I have a lot of meetings to attend so this could be useful for my note taking while at the meetings. I do have some suggestions that I think could make this even more useful:
Have the form remember my info using cookies. This way when I return for more notepaper I don't have to refill it out. Along those lines prefilling the date field with today's date would also be useful.

Have you thought about doing a 2 page pdf for output for two sided note taking? While I can't see myself using this daily to print out note paper I can see myself putting the pdf onto a flash drive and having office depot printing me out a batch all at once. Having two sides to take notes on would be pretty effecient use of the paper I'd think.

On second thought T - unless I'm misunderstanding something - all you would need to do is tell the kind associate at your printer of choice that you want it printed double-sided.
The only thing I can see that would need to be changed is the hole punch guides would need to be moved. I'm guessing that if you get a bunch of these printed and you want holes, you would probably spend the extra $5 to get a three hole punch also.
Am I pickin' up what you're puttin' down?

I take math, physics, and electrical engineering notes and often need to sketch or graph something. I hate to take notes on grid paper, so I normally use narrow-ruled notebook paper and slip a rotated blank sheet behind it to let me draw a graph. This is an imperfect solution and I would enjoy a sheet where the area for notes had horizontal lines with vertical lines printed on the back to form a graph.

as for Trachalio's 2-sided comment, it would be really convenient to have the option to switch the cue column to the right side, because, while taking notes live, the rings of a binder can get really annoying really quickly. basically, just a mirror of the page as it already is for front/back printing.

Mega props/kudos to this web app...I love graph paper, but I'm also a nerd who likes custom graph paper!
As a suggestion to those looking to print in quantity and with the mirror image idea...if you had a pdf with a mirror image for page two you could take it to your local copy shop, have them print off 100 sheets, place a heavy plastic cover on the front and back, add a spiral and *poof* you have a custom notebook! My friend did this with graph paper for me last year for a custom journal. I love it

I've made some updates (including A4 for all my friends elsewhere). Please take a moment to test them out and try to think of anything else you might like to see this little app do.

Now this is what I'm talking about. I'm a college student with a very unorganized free note taking skills. I'm reading and taking notes more then ever and still miss a few questions on tests. I think I checked this out over the summer and was mildly amused, but with the option of Ted Style I think I'm digging it.
Here is my suggestion, for my name and class name I just threw down "__________________" That way I can put the date, page num., etc up there.

Nice work and quickly too with the A4 options.
Now if you wanted to get really fancy and international, the rest of the world uses 2 (or sometimes 4) hole punches. This is not as big a deal as the paper size, as my hole punch knows where to make the holes , but for completists...

Great idea, and perfect for taking notes in meetings at work.
The only thing that would improve it for my use, is too disable the name/class/date on the 2nd page of a double sided print. Currently I just print a blank one and a personalized one, and have the photocopier merge them into a double sided document. It would be nice to just have the printer do the work in one go.

Great page, works well in Firefox but instantly crashes Safari - all I have to do is select the Graph checkbox and Safari crashes...

Nice work, a great page that I'm passing on to my biz friends. One side note: this is the first page in months that has crashed my browser (Safari v2.0.1). Just crashes it dead once I hit the "Ruled" radio button. But it worked fine in Firefox.

Thanks to everyone for all the comments. I'll check on that Safari thing but from the two comments that mentioned it, it sounds like there is a bug in the javascript that causes safari to crash. Unfortunately, I don't have a mac to test on so I don't know when I can fix this bug.
Second, I'll add 'cm' to my 'to-do' list for this app as well as a Franklin Planner version.

I'm going to teach 8th graders the Cornell method and your template was great! How about adding a column for illustrations? You can increase your learning by 20% - 40% by creating nonlinguistic representations of what you are studying!

i do both typically. i usually use this sheet for lectures and use the summaries from them to reiterate what i need to re-read from the book. as for plain-ol-book-studying, i typically write in the book or create an outline of the passage depending on what im studying.

I've been looking for something like this ever since I first learned of the Cornell method a week ago. Unfortunately, despite the claims in Pauk's book, NO ONE sells Cornell-style notepaper or notebooks.There is more evidence that Saddam had weapons of mass destruction. The closest I've come across is the legal stlye ones, except the margin is more like 3 1/2 inches...not enough room for note taking. Its a pain getting a ruler and doing it by hand....contrary to Pauk's point of budgeting time.
I am assuming the best way to do this is to use a lined version(for taking lecture notes in college)However I don't know how many lines per inch I should use to mimic the college ruled 8 1/2 by 11 college ruled notebooks.

Nice work! I'd like the option to remove the blank section at the bottom (the summary section), or only have it print on the last page or some such. I"ve found many people prefer this "modified Cornell" system.
I really dig this. I admit, it's not that much more useful than a wide margin legal pad, but so much cooler.

Good idea, well implemented. I would like to see the option of printing lines or graphs paper in color, particularly medium blue, and adjust the darkness of that. If you can get around to it, I'll use it, for sure! I find a heavier weight of paper to be nicer to write on, too (higher than 20, which is standard copier/printer paper).

Haloscan (my comment manager) deleted the first 15 or so comments for whatever reason. Ted Style is pretty simple really. It prints horizontal lines on the front of the page and vertical lines on the back thus creating a quasi graph paper because you can see through your page. I'd encourage you to try it out. I think you'll like it.

The "Date" field is an unrestricted text field. Feel free to put anything there you'd like . . . "Week of April 17, 2006" for instance.

I had never heard of this notetaking system, until I asked an aunt of mine that lives in the US about how I could increase my productivity in university.
My only problem was that over on this side of the worl (I live in Chile), there are only your standard wire-bound notebooks available. This tool here really came in handy!
What I tweaked a little was the fields, instead of using them as they were intended, I made them with prompts, like this:
Subject: ________________
Date: ___/___/_______
Page ___ of ___
That way I could use it for any subject, and I didn't mix up the pages!

A little suggestion - or really, an addition.
Maybe in the advanced options, have some options so you can fill in more than just the class/name/date? I was thinking more of an "I" layout, where the whole top is sectioned so you can put class/name/date on the right side, teacher's name/page number/another info on the other side. I really would like to be able to put info on the left side of the page as well.

This site is great, my notes will never be the same, especially for science. Some ideas are just better expressed as drawings (i.e. sections of the brain).
Could you allow the user to switch around the order of the name, class, and date? Well, unless it's one of those special notetaking things, because I usually write the topic as a header.

I have one small request (on that I have seen reqested before on here). Is there a way to make it so that the user can choose to have the main note area on the left and the smaller recall area on the right?
I am left-handed and having the main note taking area on the right is not as natural as having it on the left-hand side. If this were to be implimented, then I could choose to have the page printed on 2 sides, and only use the "back side" so that the rings of the binder will not be in the way of my arm.

I just wanted to thank you for a great tool! I use it DAILY and have even impressed cute chicks with it (no lie).

I like it very much! One request, when I make 4 holes I could use some guides:
- in the middle between the first and second hole, and
- in the middle between the third and fourth hole.
Otherwise it is hard to do it precisely.

Great page, but one request. My printer prints on both sides as if it were opening a book on the left. As such, if I create a two sided page in your program, the back page comes out reversed. OK, that was complicated, but the bottom line is that I could really use a version that simply had two identical pages, on after another. Then my printer could do its thing. I realize I could just print one page, flip the paper and print again, but I can never quite figure out what direction to put the paper. Just a thought, though.

One addition that would help me out (and maybe others!) would be to have another vertical line separating the left edge of the paper from the beginning of the cue space, to allow people who don't want the punch holes to be printed to know how far over to start writing their cue comments.
Without such a border, someone without punch holes might write down comments in the cue column, and if they decide to punch holes afterwards, might punch out part of the written text.

great idea, thanks. I'm a 50 year old mum who's studying a Diploma in Marketing after leaving school at 15 years old.
I have ADHD & found this style of note taking in an old text book about 8 years ago & found it suits me especially when used with a 4 colour pen.

Next question: Are you interested in doing paper for kids with handwriting problems? Spacing that allows the kid's natural size, and a variety of line / dotted-line and weight sequences.
Given what you've done here, it might be fairly easy for you. I did them in MSWord for my son, but to change the spacing you need to know how to use that program. HTML produced unexpected (well, given the "standards", maybe not unexpected) results.

Being an engineer, I like the graph paper option in the notes section. I'd like to offer a couple of suggestions for improvements.
1. Dark grid lines - on 'standard' engineering paper, there are 5 squares per inch, but there are also darker lines that form larger 1-inch squares. Can you add an option for two line widths, one for the interior lines and one for the every 'n' lines? Can you add an option to specify how often a darker line should be printed (every 'n'th, in my case 5th, line).
2. Titles for each section - for beginners it might be nice to 'label' each section (notes, cue words, summary) so they remember what to do with each one. You could go all-out and allow the labels to be changed as well.

Very nice.
Just discovering this from Lifehacker.
One suggestion that takes the advantage of Ted Style, but lets you use two sides...
Make regular ruled lines on the front, with a small dot between each line, so that the dots form columns at the same measured interval as the number of rule lines.
This could be printed front and back. If you don't visualize it, think of sheets that have just dots instead of grids. This would be a hybrid of dot pages and ruled lines.
It would let Ted (and me) draw squares and pictures, with the dots being enough of a suggestion of the vertical grid space.
Also, you only use one shade of gray now. The dots could be one shade lighter than the rule lines.
Hope you integrate my feedback.

Expanding on the "Ted style" for physics/engineering notes, most engineering notepads actually keep the entire front page blank and have the grid on the back. That way, when you're handling a single sheet, you can barely see the grid lines, but if you have multiple sheets, you get a crisp and clear set of lines to use.
