Hi Tom, I've only been pitched once to post something on my website, so I'll provide my perspective as someone who pitches bloggers more and more as part of my day job.
You're absolutely right. I think the most common mistake is that people get lists of top blogs in a specific category and start sending form letters. Regardless of traditional media or bloggers, you have to personalize your correspondence. With bloggers, the informality of the blog itself may be mistaken as being more "easy" to touch base with a blogger.
The blogger needs to 1) see what value your information brings to their readers and 2) how you can help those bloggers. And one more thing, don't always expect to get something in return for engaging with a blogger. Building a relationship is just as important as having something written about you - you never know, they may forward to a friend who then blogs about you.
And as you mentioned, in my role at ON24, I know linklove is key for bloggers. We will include a blurb of any blog coverage on our website at least 99% of the time.
