The Mitteler Rebbe named his youngest daughter Chaya Sarah even though she already had a living older sister Chaya Mushka and a sister (who passed away young) named Sarah.

I would have to challenge your facts about the Mitteler rebbe having daughters with the same name while both were alive:It is basically unheard of even by sephardim who will name a grandson after a living granfather.The Horenstein case is different since I believe they were half sisters, probably named by different fathers

The Toldos Maharash written by the 7th Lubavitcher rebbe lists the children of Moshe Horenstein and his wife, daughter of the Maharash. This book has been reprinted by Kehoth.
If my memory serves me correctly the father of Moshe was Zalman Horenstein not Gur-Arie Horenstein , but I may be mistaken.
In the full length biography of the Boyaner rebbe of Czernowitz there are several VERY interesting page notes concerning the Horenstein family including much geneological data. At one point I put together a tree of the Horensteins of the last generations and their marriages to the Schneersohn and Friedman (Rizhin) dynasties. I'll see if I recall what I did with it.
The Horensteins were Boyaner chassidim and controlled the sugar industry in Czarist Russia.
There was a Horenstein in the US related to the Schneersohns. It was at his funeral that the photo of the rebbe and the late Boyaner rebbe walking hand in hand was taken. I think his name was Shlomo Horenstein.
Some siblings of Mendel Horenstein made it to Palestine and have great grand children living there in Israel presently.
Just a nother note the Gourary family is not related to the Horensteins and the first name GUr Arie is a cincidence. I have heard of people having that first name in Brooklyn who most probably are not related to the Horensteins.

ANSWER: The information I have from two different family sources, one of whom is his son Lipa, is that, although he had the title of rabbi, he did not use the HaKohen appellation. Moreover, none of his male descendants appears to have done so.

(It seems that the family used the HaKohen appellation before surnames were mandated in the early 19th century. Afterwards, the use of the appellation gradually faded out – although it was more likely to be used by the most scholarly and religious family members.)

QUESTION: Were all of these related Horensteins Kohanim, or were some not?

Answer: All the related Horensteins were Kohanim – although I know of other unrelated Horenstein families who were not.

QUESTION: Two Horenstein sisters with the same name (Sarah) while both were still living?

ANSWER: Good question! Your inquiry caused me to spend several hours reviewing my sources and I believe I solved the mystery of 2 siblings named Sarah. I am now convinced that the wrong R. Moshe Horenstein was listed as the husband of Chaya Mushka Schneerson. Here is the (I hope) corrected info on the Horenstein-Schneerson marriages:

1. About 1891: Shalom Noach Dulikaza, son of Sheina Bracha Schneerson, daughter of R. Yosef Yitzcock Schneersohn of Ovruch, married Hilda Horenstein, daughter of R. Israel Yakov Horenstein and Chaike Horenstein.
2. 1892: Chaya Mushka (Moussia) Schneerson, sister of R. Shalom Dov Ber (the 5th Lubavitcher Rebbe) married R. Moshe HaKohen Horenstein (b. 1869), son of R. Zalman HaKohen Horenstein [Schneur was correct] and Ester Hinda Landau.
3. 1932: Sheindel Schneerson, daughter of the 6th Lubavitcher Rebbe (R. Yosef Yitzcock Schneerson) married R. Menachem Mendel HaKohen Horenstein, son of Chaya Mushka Schneerson and R. Moshe HaKohen Horenstein.

Since R. Menachem Mendel HaKohen Horenstein’s father was listed incorrectly, so were his siblings, as only one marriage is listed for the correct R. Moshe Horenstein. I now list four siblings: Sarah, Rachel and Shmuel (who had children) and Leah (who died in her youth) – so there were not two siblings named Sarah after all!
