Just as a comment, I think if people want to kill themselves by smoking..that's their business. But I don't think it's unreasonable to ask them to step outside or do it in some area where it doesn't adversely effect people who don't share their death-wish.
I agree, music is too loud at times. There should be control over loudness in public places. Though clubs may argue: don't come if you don't like 'loudness'. Afterall, they do not force you to be in club.
