Con Ed went through the same thing in Chicago a few years back during a heat wave. Investigation revealed numerous problems with maintenance and failure to upgrade facilities and some cooking of the books along those lines. A couple of semi large heads were shifted around and a plan to alleviate the problems was implemented. I don’t think the final report was ever made 100% public to avoid embarrassing certain politicos and the real company culprits.
